uAvionix Corporation (“uAvionix”) requests that the Customer complete this Certification to help fulfill its obligations under U.S. Export Administration Regulations (“EAR”) and other U.S. trade control laws and regulations (collectively, “U.S. Export Control and Sanctions Laws”). Insufficient or incomplete information on this certification will delay your procurement. uAvionix appreciates your cooperation and your complete and accurate responses to the requests below.

The information supplied below will be kept confidential and used for determining export requirements. The information provided may be used to support uAvionix’s export license applications, if required. This information will be kept on file by uAvionix for 5 years from the date of last shipment against the specified project registration stated below. In this certification, the term “uAvionix Item” includes, but is not limited to, hardware products, parts and components, software, technology, data, and services that are supplied by uAvionix.

The information provided on this Certification may be used in the preparation of an export license application to the U.S. Department of Commerce, or another U.S. agency, if uAvionix determines that such a license is required. By completing this form, the Customer is consenting to the disclosure of the information to the appropriate U.S. agency.
